Valentia's CareMonX™ Chronic Disease Assistant facilitates automated data collection and transmission systems for individuals with chronic diseases.

Underpinning our CareMonX™ Chronic Disease Assistant is Valentia's sophisticated CareMonX™ Mobile Messaging Hub & Tool Kit, which enables “always-on” automatic data transfer between the patient and caregivers involved in the management of the patient's condition - including healthcare professionals, public health nurses, pharmacies, chronic disease organisations, family and friends.

CareMonX™ Chronic Disease Assistant facilitates transmission of a patient's clinical data (such as blood pressure, blood glucose or peak flow readings) to a central secure Clinical Messaging database, where the patient's healthcare provider can use this data for patient monitoring and early detection of problems. The CareMonX™ Chronic Disease Assistant can also be used for automated medication reminders, automated repeat prescription ordering, dose adjustments, medical reviews, distribution of public information bulletins and sharing of information with other patients with chronic diseases in an online community (with appropriate consent).

Information can be transferred from a wide range of sources including medical devices (such as glucometers, weighing scales or peak flow meters), specialised electronic sensors, mobile phones, PDAs, online and desktop based applications, touch-screen terminals and digital pens. Together, these systems provide a complete communications network between patients and their caregivers and amongst multiple healthcare professionals who are caring for the same patient.

Healthcare professionals can view patient data in appropriate graphical formats via desktop, PDA or internet browser and respond appropriately. This system facilitates the management of substantial numbers of dispersed patients at single or multiple locations and enables practitioners to detect and manage potential problems before a hospital admission is required.

CareMonX™ Chronic Disease Assistant is also complemented with online patient portal that provides the patient with easy-to-read graphs and statistical data on their clinical results. Combined with regular feedback from the patient's healthcare team, this facility can enable patients to become empowered to manage their own condition.

CareMonX™ Chronic Disease Assistant is capable of interoperability with hospital patient administration systems and other clinical databases and thus all captured data can become part of each patient's permanent Electronic Health Record.
