CareMonX™ Integrated Care

CareMonX™ Integrated Care provides infrastructure and services for the provision of in-community health and social services.

Until recently, the traditional focus of healthcare technology has been on injury, disease, and disability. However, as ageing populations increase worldwide, healthcare costs are rising and the focus is shifting towards healthcare technologies that will facilitate disease prevention, promote healthy living and empower elderly or disabled persons to live independently.

Valentia's CareMonX™ Integrated Care Platform facilitates the creation of enhanced independent living networks for individual's healthcare and social needs. In addition to the external networking services CareMonX™ Integrated Care also supports collection of data from motion sensors, wireless biomedical sensors, environmental sensors, medical devices, microcontrollers, cameras, alarm buttons and other associated devices. This data can be transmitted to an intelligent automated monitoring and alerting system where it can be used to interpret events within the house and trigger alerts in the case of unusual activity (such as falls, increased bathroom visits or forgetting to take medication). Communication back into the home is also possible using GSM-enabled touch-screen terminals or mobile phones.

Underpinning CareMonX™ Integrated Care is Valentia's sophisticated health messaging hub and services bus, which enables “always-on” automatic data transfer between devices in the patient's home environment and all others involved in the management of the patient's condition – including home monitoring centres, healthcare professionals, public health nurses, intelligent automated alerting systems, pharmacies, elderly care organizations, family and friends.

CareMonX™ Integrated Care assists individuals who may have a diverse range of requirements – from active, healthy elderly subjects who require minimal assistance to subjects with moderately severe physical disabilities or memory impairments. The CareMonX™ Integrated Care system can be used for general monitoring of activities within the home and also medication compliance, falls detection, cognitive engagement, physical fitness maintenance, nutrition and social activity. Thus an elderly or disabled person can function independently in their own homes (or in a managed healthcare setting) for as long as possible.

Care Team (Caregivers, healthcare practitioners, family members) can access real-time social services & diagnostic data based on subjects' activities through a remote monitoring station, mobile phone, PDA or an online portal. Care Team can also interact with the subject via voice, video and Valentia's own assisted living instant messaging system.

In addition to elderly or disabled persons, Valentia's platforms can also can be tailored to empower health-conscious or at-risk individuals to proactively participate in their health management. See the Chronic Disease Management Platform section for further details.
